Create Question Sets

Question Sets are reusable groups of questions that you create once and then use across forms such as inductions, RAMS reviews, permits, and site briefings. They are useful for keeping forms consistent and reducing setup time when creating a new UK account.

What this means

  • A “Question” is a single prompt (for example, a Yes/No question or a text answer).
  • A “Question Set” is a reusable set of questions you can add to multiple forms, so you don’t have to rebuild the same questions each time.
  • You can also set “display conditions” so a question only appears when someone answers a previous question in a certain way (conditional logic).

When you’ll use this

  • When you want the same questions reused across multiple forms (for example, repeated site safety questions)
  • When you’re creating standard UK templates (site induction, visitor induction, RAMS review, permits)
  • When you need conditional follow-up questions (for example, “If yes, please explain”)

Step-by-step

    Common issues

Question Set doesn’t show up when editing a form

    What to check:

    • The Question Set is saved and active (not inactive).
    • You have permission to access Editor: Questions and edit forms.

    What to do:

    • Make the Question Set active and try adding it again from the form editor.

Follow-up question always shows (conditional logic not working)

    What to check:

    • Display Conditions are set on the follow-up question (not the parent question).
    • The condition matches the exact answer option (for example, “Yes” vs “yes”).

    What to do:

    • Re-open Display Conditions and re-select the parent question and correct answer rule.

You have duplicate questions across forms

    What to check:

    • Whether you created slightly different versions of the same question instead of reusing one.

    What to do:

    • Standardise on a single “approved” question inside a Question Set, then replace duplicates over time.

    Why this matters

    • Question Sets reduce setup time and prevent inconsistencies across sites and forms.
    • Conditional logic keeps forms short and easier to complete, while still capturing details when they matter.

    Need help?

If you’re not sure how to structure a Question Set, start with a small “core set” of questions you know you’ll reuse, add it to one form first, and test the flow end to end. If you need conditional logic for a specific workflow (for example, RAMS review comments triggered on “No”), build and test that pattern once, then reuse it across similar forms.
